Instructions for filling out the form N 1-OL

federal statistical observation N 1-OL is provided by legal entities of all forms of ownership that provide services for the organization of children's recreational activities:

State-owned (federal or property of a constituent entity of the Russian Federation);

owned by the municipality;

Maintained by registered balance holders legal entity on the territory of a subject of the Russian Federation.

A legal entity fills out this form and submits it to the territorial body of Rosstat at its location.

In the case when a legal entity does not carry out activities for organizing children's recreation at its location, the form is provided at the place of actual implementation of the activities of the legal entity.

If a children's health-improving institution (camp) does not have the status of a legal entity, but is its separate subdivision and is kept on the balance sheet of a legal entity located on the territory of another subject of the Russian Federation, the form is filled in by the legal entity and submitted to the territorial body of Rosstat at the location of the legal entity.

The head of the legal entity appoints officials authorized to provide primary statistical data on behalf of a legal entity to the territorial body of Rosstat.

The address part of the form indicates the full name and type of a children's health institution (camp) in accordance with founding documents, registered in the prescribed manner, and then in brackets - a short name. On the blank of the form containing information on a children's health institution that does not have the status of a legal entity and is a separate subdivision of a legal entity, the name of the subdivision and the legal entity on whose balance sheet it is contained is indicated.

The line "Postal address" indicates the name of the subject of the Russian Federation, the legal address with a postal code; if the actual address does not match the legal one, then the actual postal address is also indicated. For children's health-improving institutions (camps) that do not have the status of a legal entity and are contained on the balance sheet of legal entities located on the territory of other constituent entities of the Russian Federation, the postal address of the legal entity with a postal code is also indicated.

A legal entity affixes the code of the All-Russian Classifier of Enterprises and Organizations (OKPO) in the code part of the form on the basis of the Notice of assignment of the OKPO code sent (issued) to organizations territorial bodies Rosstat.

According to the form N 1-OL, information is filled in on the activities of a children's health institution (camp) for students educational institutions: out-of-town health-improving, health-improving, health-improving with day stay, work and rest, tent. Health-improving institutions can be summer and year-round functioning; the latter are only accountable for summer period work on the date of its closing (no later than September 1).

The form contains all the required indicators. In the absence of any data, a dash is put in the corresponding graph cell.

Children's health-improving institutions (camps) for students of educational institutions are created by: state power Russian Federation; bodies exercising management in the field of education; bodies of social protection of the population; youth committees; voluntary sports societies; sports committees; trade union bodies; other bodies of the subjects of the Russian Federation and local government; commercial and non-profit organizations and are divided into the following types:

1. Out-of-town health camps are stationary and are organized for children aged 6 - 15 years (inclusive);

2. Sanatorium camps. Health-improving camps, as well as health-improving groups for children, open only in summer shifts at year-round children's and adult sanatoriums, in which summer rest children (with the appropriate permission of the territorial department of Rospotrebnadzor). Children's sanatoriums do not provide form N 1-OL;

3. Health camps with day stay are organized at the base educational institutions, institutions additional education children, sports facilities, parks of culture and recreation and other institutions. The duration of the stay of children in the camp can be up to 14 hours 30 minutes with 2 meals a day and up to 18 hours with 3 meals a day;

4. Labor and recreation camps are organized for students who have reached the age of 14, are created with a round-the-clock or day stay. Labor activity they are combined with recreational and educational activities;

5. Tent camps are organized for students of educational institutions aged 12 years and older (in exceptional cases, children from the age of 10 are allowed to enroll in tent camps) for the purpose of their recreation, practical acquisition of skills in natural conditions, classes physical education, sports, tourism.

A children's health institution should be accepted for opening by territorial sanitary and epidemiological services (territorial department of Rospotrebnadzor).

A children's health-improving institution may be located in a specially built or rented for this purpose premises, tents.

On the basis of a health camp of any type, a profile shift (shifts) can be organized, combining recreation and health improvement of children with the organization of activities in order to deepen knowledge, skills in scientific, technical, artistic creativity, sports, tourism and local history, in other types cognitive activity.

Profile health-improving institutions (camps) refer to one of the 5 types of health-improving institutions (camps) mentioned above, on the basis of which they are organized.

When filling out section 3, the name of the subject of the Russian Federation is affixed in accordance with All-Russian classifier administrative-territorial division in ascending order of their codes.

When filling in the code of the subject of the Russian Federation for the republics, territories, regions, cities of federal significance, autonomous region, the first 2 characters are affixed, according to autonomous regions- 5 characters.

Column 1 indicates the name of the subject of the Russian Federation, in column 2 - the line number, in column 3 - the code of the subject of the Russian Federation according to OKATO, in column 4 - the number of children who arrived from each subject.

In column 5 of total strength children, the number of children aged 15 and over is allocated.

Column 6 reflects the number of children whose vouchers are paid entirely at the expense of their parents (legal representatives).

When completing section 3, the following ratios should be taken into account:

1. Data for line 01 and each line disclosing information on the number of children for the constituent entities of the Russian Federation:

column 4 must be greater than or equal to column 5;

column 4 must be greater than or equal to column 6.

2. The sum of these lines, disclosing for the constituent entities of the Russian Federation information on the number of children who rested in a children's health institution over the summer, in columns 4 - 6 should be equal to line 01 in the corresponding columns.

3. Line 01 section 3 data:

columns 4 must be equal to the data of line 01 of column 3 of section 2;

columns 5 must be equal to the data of line 02 of column 3 of section 2.

In accordance with subparagraph 5.5 of the Regulations on the Federal Service state statistics, approved by Decree of the Government of the Russian Federation of June 2, 2008 N 420, and in pursuance of the Federal Plan statistical works, approved by the order of the Government of the Russian Federation of May 6, 2008 N 671-r, I order:

1. Approve the attached form of the federal statistical observation "Information on the organization of children's recreation and their rehabilitation" with the completion of it and put it into effect once a year, starting with the report for the summer period of 2018.

2. Establish the provision of data at the federal statistical observation indicated in this order at the addresses and within the time limits established in the form.

The code for the type of settlement is affixed:

city ​​and urban-type settlement - "1"; countryside - "2".

When filling out this line, one should be guided by the place where the rest and recreation of children is actually organized.

Section 2. Distribution of the number of rested children, by the constituent entities of the Russian Federation from which they arrived

C indicates the distribution of the number of rested children, according to the constituent entities of the Russian Federation from which they arrived, in ascending order of their codes according to the All-Russian Classifier of Objects of Administrative-Territorial Division (hereinafter referred to as OKATO), including the constituent entity of the Russian Federation in whose territory the organization of children's recreation and their rehabilitation operates if children living in the same territory rested in this organization. The number of children is determined according to the registration log of the arrival and departure of children from the organization of children's recreation and their rehabilitation, filled out on the basis of agreements on the organization of recreation and rehabilitation of the child, concluded by the organization of recreation for children and their rehabilitation with the parent (legal representative) of the child sent for rest and rehabilitation in the specified organization, and (or) vouchers or an administrative act for the organization of children's recreation and their rehabilitation on the enrollment of children in the specified organization. Moreover, if one child was in the organization (organizations) of children's recreation and their rehabilitation two or more times during the year, then he is included in the report twice (several times).

In the total number of children (from column 4), the number of girls is given.

In the total number of children, children with disabilities are distinguished, in column 8 - children with disabilities.

Article 2 federal law dated December 29, 2012 N 273-FZ "On Education in the Russian Federation" a person with disabilities - individual having deficiencies in physical and (or) psychological development, confirmed by the psychological-medical-pedagogical commission and preventing education without creating special conditions.

In accordance with Article 1 of the Federal Law of November 24, 1995 N 181-FZ "On the Social Protection of Disabled Persons in the Russian Federation", a disabled person is a person who has a health disorder with a persistent disorder of body functions due to diseases, the consequences of injuries or defects, leading to a limitation of life and necessitating social protection.

Depending on the degree of impairment of body functions, persons recognized as disabled are assigned a disability group, and persons under the age of 18 are assigned the category "disabled child".

The recognition of a person as a disabled person is carried out by the federal institution of medical and social expertise in accordance with the rules approved by Decree of the Government of the Russian Federation of February 20, 2006 N 95 "On the procedure and conditions for recognizing a person as disabled." At the same time, in accordance with the legislation of the Russian Federation, the concepts of "disabled person" and "person with limited health capabilities" are not identical.

A child recognized as both a disabled person and a person with disabilities is counted as a disabled person at the same time.

Of the total number of children with disabilities (from) and children with disabilities (from column 8), girls stand out (columns 7 and 9, respectively).

B reflects the number of children sent to organize children's recreation and their rehabilitation completely at the expense of the parents (legal representatives) of the child.
